First Visit Instructions

When you come for your first visit please remember to obtain and bring the following:

• Referral letter from GP or another doctor
• Medicare card, DVA card, Pension card
• Private Hospital Insurance details
• Reports of any ultrasound, CT scan, MRI scan, X-rays and Pathology results
• List of current medications you may be taking
• Workers compensation claim approval letter and reference number (If work cover)
